How to Enroll

Click ENROLL NOW and follow these simple steps:

Demographics: the first step is entering basic demographic information for your child.

Payment: Once demographic information is entered, you will enter your payment information. We prefer bank payments as there are significantly lower fees for our practice, but we also offer the option of using a credit card.

Medical History: Next, you will complete the medical history for your child and/or family if there are any notable medical issues for parents, grandparents or siblings. You can skip this tab if there is nothing pertinent to add. Some questions are geared more towards adults, and you can skip those. At the bottom of the health history page, there is small text that says, “skip to the last step” which you can click if you want to bypass the health history pages. You can skip vaccine history if your child’s immunization history is available on Florida Shots.

Welcome Links: Finally, you will receive a welcome email confirming your enrollment has been accepted. If you have medical records we need to request, please notify us of that when you receive this email so we can send you a records release form.

If you have private insurance, you will also need to register with Vaxcare. To do that, go to this link: https://patients.vaxcare.com/registration/

Use Code: FL26071 to register for the Honeybee community clinic. 

When you get to the end of registration, there will be a statement that says “I have read and agree to the Terms of Service for this clinic. Please click on “Terms of Service” and review the patient agreement. You will then check the box next to that statement to move forward with enrollment.
