How to Enroll

Once you have received approval from the practice to enroll, please follow these instructions:

•Choose the appropriate enrollment link. If you are unsure which one you need, please reach out to the practice to confirm.

For Concierge Care Membership click HERE.
For Essential Care Membership click HERE.
For Fourth Trimester Honeycomb Membership click HERE.

•Choose if you would like to pay monthly or annually.
•On the next page, enter the responsible party’s information, then click the toggle button that says, “I am enrolling others, but not myself.”
•Click SAVE, then on the next page, click ADD.
•If you are registering multiple children, once you save the information for the first child, you can save and click ADD again.
•If you are still pregnant, you will need to enter the date that you are signing up as the baby’s DOB. Once your baby is born, we will edit that date to the correct one.
•Once you are finished entering children click CONTINUE.
•Next, choose a start date for your membership. This date should have been given to you by either Angela or Dr. Coleman when they gave you approval to register. If you are having a new baby, this date will be he 1st of the month of your due date, unless your due date is the last week of the month, then you will choose the 15th of the month.
•You will then enter the payment information and review the medical records release form and membership agreement, then complete all other necessary forms.
•You will be required to review the entire membership agreement and then type your name in the box to indicate acceptance.
Then click PAY AND CONFIRM.
•If paying monthly, the registration fee will be charged at the time of registration. Your first payment will occur on the date chosen for the start of your membership.

Once you are finished with the enrollment process, please download the Sigma app and enter insurance information on your child’s profile if it is available.

If you have private insurance and will be getting vaccines, you will also need to register with Vaxcare. Click here to register.
