•Comprehensive care coordination and advocacy—we will assist with scheduling appointments with specialists and therapists and follow up after visits.

Monthly Pricing:

1-12 months: $195 per month
12-23 months: $150 per month
2-4 years: $135 per month
5 years+: $135 per month
Family maximum: $500 per month

*Families who pay annually will receive a 5% discount.

*Discounts are available for families with children in foster care, teachers, medical students, and residents.

*A limited number of financial assistance spots are available to families with an annual household income of less than $60,000. Please contact us for more information.

*A limited number of openings are available for one-time visits for various needs. Please contact us to discuss availability and pricing.

*Essential membership patients who transition to a Concierge Care membership will not pay a registration fee and will receive a 10% discount for the first six months of membership.

*House calls outside a 20 minute drive from the office will be charged an additional fee based on distance.
